Services are individualized through assessment, provider capability review, applicable orders and the resident’s service plan.

Personal care and daily living

Assistance may include bathing, grooming, dressing, toileting, continence support, eating and other activities of daily living based on assessed need.

Medication and health-related support

Medication management or administration, ordered monitoring, treatments and provider coordination may be available when appropriate to the resident’s needs and Evergreen’s service capability.

Mobility and transfers

Support may include walking, wheelchair mobility, transfers, bed mobility and individualized fall-prevention routines.

Cognitive and behavioral support

Evergreen provides structured support, cueing, reassurance, redirection and individualized interventions for residents who benefit from cognitive or behavioral assistance.

Meals, household support and engagement

Home-style meals, snacks, hydration, housekeeping, laundry, linen service, activities and community participation are integrated into daily life.
