Understanding assisted-living payment pathways.
Evergreen accepts multiple payment pathways. The exact process depends on the resident’s eligibility, benefits, authorization, requested services and placement fit.
Private Pay
Private-pay placement typically begins with a discussion of care needs and a preadmission assessment so the proposed service rate can reflect the level of assistance required.
Long-Term Care Insurance
Residents with applicable long-term care insurance may use benefits toward eligible assisted-living expenses according to the terms of their policy. Evergreen can coordinate requested provider documentation when appropriate.
CADI Waiver
Community Access for Disability Inclusion referrals may be considered when the resident is eligible, the requested services and setting are appropriate, and authorization requirements are met.
Brain Injury Waiver
BI waiver referrals are reviewed for service capability, safety, behavioral or cognitive support needs, and applicable authorization requirements.
Elderly Waiver
EW referrals are reviewed when the resident’s needs, setting and requested services align with Evergreen’s provider capability and the applicable payer requirements.
Important: payment-pathway acceptance does not guarantee admission. Every prospective placement requires an individualized fit and service-capability review.
