Understanding common payment pathways for assisted living.
Payment is only one part of placement. Evergreen also reviews eligibility, authorization, assessed needs, provider capability and overall fit.
Private Pay
Private-pay placement generally involves reviewing the resident’s assessed service needs, housing arrangement and proposed service rate before admission.
Long-Term Care Insurance
Coverage varies by policy. Families should review benefit triggers, elimination periods, covered services, documentation requirements and claim procedures with the insurer.
Medicaid waiver programs
Evergreen accepts Medicaid waiver programs including CADI, Brain Injury (BI) and Elderly Waiver (EW), subject to the person’s eligibility, authorization, service needs, provider capability and placement fit.
For case managers
Include payer/program information, county, case-manager contact, authorization status and relevant service-plan information with the referral whenever available.
